PJ MEDCAST are the podcasts for PJ MED (Pararescue Medicine). Besides PJs, these podcasts may be useful to other Military, Law Enforcement, Federal Agency and Civilian Medical Personnel involved in Tactical and Technical Rescue Medicine, and other facets of Operational Medicine. PJs may document the time they spend listening to podcasts as training for their training folders, and also apply towards Paramedic Recertification. This podcast represents the positions of the authors and may not represent the offical position of the Department of the Defense, US Air Force or the United States Government.

A recently deployed and very experienced DOD trauma surgeon just came back from a deployment and shares observations and thoughts on overuse of certain techniques and emphasizing know the indications and technical details of performing invasive procedures.

These procedures should be used when indicated, and not used when not indicated. 
There are potential adverse consequences of performing procedures. The downside is minimized when they are not performed if not indicated.

Although tension pneumothorax has declined as a cause of potentially preventable death, needle decompression should only be used when indicated to reduce unnecessary complications. Excellent technical performance through rigorous training is the other way to reduce complications.

Notes on trauma from the ATLS course- advanced trauma life support. Although many principles are similar, to what we do in the field,  there are some differences how to handle trauma patients immediately in an ER from what we do. 

This podcast reviews some trauma pearls and gives you insight into how an ER or forward MTF handles trauma, stabilizes patients, and evaluates the need for immediate transfer to a higher level of care.

The new PJ Medical Operations Handbook is posted on the GA Sharepoint.

ACC is determining if there will be a mass printing.
In the meantime Teams can print them locally.

Recent missions with lessons learned on thoracic trauma are briefly discussed.

The med cert has been updated and made more user friendly.

Finally- with great sadness for those of you who have come to PJ Med Courses, my friend and Cardiothoracic Surgeon Dr. Michael Graver and his wife Jodi Cohen, friend of the 103rd and the Pararescue career-field and pilot, died this week in a small plane crash. Dr. Graver took time out of his schedule several days a year to volunteer to teach PJs about thoracic trauma in the Cadaver lab. He was committed to teaching PJs and was always proud to read about PJs hacking the mish around the world.

Part 3 of 5. Operation Wild Boar.

Another exceptional discussion of the evolution of this incredible mission that gripped the world.

Today is 9-11-2018.

On behalf of TEAM NY, the 103rd Rescue Squadron, we send our warmest wishes to the survivors of the citizens, FDNY, NYPD, PAPD members and others who perished on 9-11.  We will never forget.

PJs from the 103rd were on the pile with the FDNY firefighters on September 12.

We are grateful for our Operators who stay vigilant in continuing to protect our freedom, and our law enforcement and first responders who are exceptional Americans we count on everyday.

He discusses the issues of high ops tempo, toll on the family, and details about the R & R program Doc Bryan discussed last week.

We really want to drive home the message that this is appropriate for burn out and smoldering issues, not just more definitive mental health issues like PTSD and Depression.

Doc Bryan discusses this focused 2 week intensive program based out of Park City and Salt Lake City Utah. Comparable or better results compared to standard treatments that take months. 

Several PJs and other SOF Operators have used this with success. 


It is also unique because the families can come and take part in discussions and therapy if indicated,  and also enjoy all that Park City has to offer in all seasons.

 
Treatment is free and the PJ Foundation generally covers ancillary costs.

Doc Bryan is a prior AF Psychologist. He currently is the Executive Director of National Center for Veteran's Studies at the Univ of Utah. He partnered with the AF Psychology Service to perform long term research on our Operators during OEF.  

The articles are posted on pjmed.com

In this episode he discusses findings that have been published in peer reviewed journals, and implications for PJ Mental Health.

He also runs a two week program which we will highlight in another episode.
